I can provide a real-life example of this, or pretty close. Once upon a time, when I was working in computer system support, we had three major work sites in our metro area. Each site had a gateway to manage traffic. While I don't recall the exact cause of the system outage, one of those gateways crashed, hard. Yes, they had been doing backups - via the network, to another server at their site. Thus, the backups were not and would not be accessible until we found some way to bring the crashed system online enough to access the remote tape drive. In order to do that, we had to load a basic operating system on the machine via its local tape drive (of a different type than the ones used for the network backup), and the only means of doing that was a physical-media tape from another system of the same type. We had such a tape, but it was old enough that it needed a root password from three iterations previous.

It's a good thing that *I* was the tech they sent onsite with the boot tape, because I was the only one of us who could remember said password! I did get the gateway back online; it took a couple of hours, and I had managers popping in requesting status reports about every five minutes - which slowed things down further, of course. But without my ability to remember that password, they would have been "dead in the water" a whole lot longer.
